Once you've booked your appointment, you will receive a confirmation email. Within that email there will be a link to complete a VITAMIN B12 CONSULTATION AND CONSENT FORM. This must be done at least 48hrs prior to your appointment date. If this is not received within the required lead-time, you may not be able to have your injection and you will forfeit your payment. If you have any problems completing the form, please let me know immediately, so as not to disappoint.
